DDLSS: Rebecka Arnsrud Godtman "Optimizing Prostate Cancer Screening: From Randomized Trials to Real-World Implementation"

Dr. Rebecka Arnsrud Godtman, MD, PhD, is Associate Professor and Senior Consultant Urologist at Sahlgrenska University Hospital, Gothenburg, Sweden. Her work focuses on prostate cancer, with particular emphasis on early detection and population-based screening. She is Principal Investigator of the Göteborg-1 and Göteborg-2 prostate cancer screening studies and a member of the steering committee of the European Randomized Study of Screening for Prostate Cancer (ERSPC). Dr. Arnsrud Godtman has played a key role in the development and implementation of organized prostate cancer testing in Sweden and currently leads the regional program in Västra Götaland. Her work integrates clinical practice and research to advance evidence-based prostate cancer screening strategies and reduce mortality.

Prostate cancer screening remains a complex balance between reducing mortality and minimizing overdiagnosis and overtreatment. The Göteborg-1 and Göteborg-2 studies have provided important insights into how screening strategies can be optimized to address this challenge. This presentation will summarize long-term outcomes from the Göteborg-1 trial, including its impact on prostate cancer mortality and overdiagnosis. The ongoing Göteborg-2 study focuses on developing screening strategies that reduce overdiagnosis while maintaining safety and efficiency. I will present results from the introduction of magnetic resonance imaging (MRI) into the screening pathway and discuss the development of risk-adapted algorithms integrating biomarkers, MRI, and structured screening pathways to improve diagnostic precision and avoid unnecessary interventions. I will also discuss potential harms of screening, including psychological impact, patient-reported experiences, and complications. Finally, I will describe how these findings have been translated into real-world practice through the implementation of organized prostate cancer testing in Sweden, highlighting key challenges in balancing benefits and harms at scale.
